Procedure performed: Irrigation and debridement of open wound. Primary repair of the 3 inch laceration. Open reduction and internal fixation of both forearm fractures, with two each six-hole DCP plates. Preoperative Diagnosis: Grade II open both bone left forearm fracture Postoperative Diagnosis: Grade II open both bone left forearm fracture Anesthetic: General oral endotracheal Indications: This 31 year old young man who finished his weeks work, drank an excessive of a six pack and while horsing around with his fellow friends, fell and injured his left upper extremity. He presented urgently for care. Operative Technique: Under general anesthesia, the patient was left in the supine position. His left upper extremity was scrubbed vigorously with Betadine and painted and draped in the standard sterile fashion. The limb was elevated and the tourniquet inflated to 250 torr. The ulnar laceration was extended proximately and distally and devitalized to subcutaneous tissue. Muscle and bone were removed. Skin margins were freshened. The fracture site was then irrigated and under direct visualization reduction of the fracture was achieved. A six hole DCP plate was then fixed to the ulnar with 3.5 cortical screws. Attention was then directed towards the radius fracture. A volar incision was fashioned. Full thickness skin flaps were elevated. Superficial veins were clamped and electrocauterized. The fascia of the flexor musculature was identified in the interval between the long and ring flexor superficialis and with the combination of blunt and sharp dissection the fracture site in the radius was exposed. With great difficulty the fracture was reduced. A six hole DCP plate was then used in a compression mode to maintain the reduction and compress the fracture site. The fracture site was scanned intraoperatively with radiographs and felt to be anatomically well aligned. Both incisions were irrigated with sterile saline. The fascia was repaired with 3-0 Vicryl and the skin with 4-0 nylon. Both fracture sites were injected with 0.5% Marcaine and a sterile compression dressing was applied. The patient was delivered to the recovery area having tolerated the procedure well. Sponge, needle, and instrument counts were reported as correct. Estimated blood loss was minimal. Tourniquet time was one hour 15 minutes. CPT code(s):
