cenario One: Catheter-Acquired Urinary Tract Infections (CAUTI) PDSA Catheter-acquired urinary tract infections (CAUTI) are some of the most common hospital-acquired infections (HAI) and are associated with increased mortality and morbidity. The main reasons for the infections are using the catheter for longer than needed and failure to properly care for the catheter. Plan: A multidisciplinary team of bedside nurses and physicians had a goal to achieve a 25% reduction in CAUTI in the ICU within the next six months with the ultimate goal of no CAUTI. The plan included: A criteria and protocol for catheter insertion Alternatives, such as condom catheter and pure wick devices, were to be considered Development of guidelines and care bundles for how to maintain the catheter including: Daily review of continued need for catheter Hand hygiene and aseptic handling of the catheter Positioning of the catheter below the level of the bladder and off the floor Regular emptying every 8 hours or at two-thirds full Aseptic technique for sampling, with sampling only at port Do: The protocols for catheter insertion and care bundles were approved. All staff members in the ICU were educated. Study: An infection preventionist rounded each day during the first two weeks of the new insertion protocol and maintenance bundle to ensure compliance and answer questions from staff. Compliance with insertion and maintenance bundles increased from 55% to 100%. The CAUTI rate was calculated monthly. The monthly rate of CAUTI dropped from 7.6 infections per 1,000 device days to 3.9 infections per 1,000 device days. Act: While greatly improved, the team realizes that other evidence-based actions can assist them in meeting the goal of zero CAUTI.
