[Verified Answer]: Task: Clinical case scenario

Task: Clinical case scenario. Learning outcomes Critically examine the implications of global and national strategies and priorities in healthy ageing, aged care and long-term conditions for the delivery of culturally safe, effective, ethical and quality health services and nursing practice. Demonstrate a practical, evidence-informed, and strengths-based approach to assessment, planning, delivery, and evaluation of nursing practice in the culturally safe care of the older person and persons with long-term conditions. Relevance The global population is ageing at an increasing rate, and the elderly population makes up the majority of patients in acute care and aged and community care services. Registered nurses must understand the ageing process and care considerations for the ageing population. This assessment provides an opportunity to apply parts of the Clinical Reasoning Cycle to identify suitable nursing assessments and a culturally safe strength-based care plan for an elderly patient. It is also an opportunity to demonstrate knowledge of contributing factors to the patient’s current health condition and the role of a strength-based approach. Case Mrs Anderson is an 84-year-old lady from a First Nations background living in Katherine. She has been referred to you as a community nurse by the G.P. to manage her pressure areas.Mrs Anderson is vulnerable to pressure damage as her skin has become more fragile and thinner with age. Following a recent fall, Mrs. Anderson has lost confidence and is now housebound, spending more time in her chair. She has a history of high blood pressure and type 2 diabetes, for which she takes medication. She has no history of previous falls or problems with her balance.Mrs. Anderson lives in a double-story house on her own. Her husband passed away 15 years ago, and she receives a pension from Centrelink. Her two children live in a city 8 hours’ drive away. Mrs. Anderson’s neighbour takes her to church on Sundays and occasionally cooks fresh food for her.Mrs. Anderson has recently had difficulty in doing her ADLs. On your visit, she appeared underweight, and you noticed her looking untidy and unkempt, wearing clothes that seemed to be dirty. Task Instructions: This assessment is in two parts. Part A: After reading the clinical scenario, identify, discuss, and rationalize THREE priority nursing assessments for the patient. (Nutritional assessment, functional assessment, Skin integrity assessment) (Approx. 800 words) Part B: Develop a culturally safe and strength-based care plan for Mrs Anderson. The care plan MUST be evidence-based, present global/national strategies, and be supported by nursing literature. (Approx. 1200 words)
