[Expert Solution]: Preoperative Diagnosis: Abnormal rig…

Preoperative Diagnosis: Abnormal rig calization History: The patient is a 68-year-old, white female who was seen by Dr. Quinn for evaluation of vertigo. During her medical workup, at that time, screening mammography was performed and showed a lump in the right breast. This was not palpable on physical exam. She denied any bloody drainage or change in breast contour or pain. It was felt that breast biopsy was indicated. Operative Findings: The needle was placed and follow-up mammography showed the suspicious area was more superior and deeper to the tip of the needle and this was taken in consideration during the biopsy procedure. During the dissection, a large, probably 2 cm. cyst was entered and had some milky fluid returned which had the appearance of lymph. No pus was encountered. No other abnormalities were noted. After removal of the mass, the wound was visually palpated and no other areas were felt. Description of Procedure: With the patient in the supine position and, after the usual monitors were placed, general anesthetic by mask was induced. Her right breast was prepped with Povidone Iodine and draped in a sterile fashion. A radial incision was then made to include the needle which had been placed by Dr. Swanson preoperatively. This was in the lower medial quadrant. The dissection was then carried down sharply through the subcutaneous tissue. Then a circumferential area of breast tissue was removed which included the needle. Following removal of tissue, the wound was digitally palpated and no other abnormalities noted. The wound was irrigated with normal saline and hemostasis controlled using electrocautery. After adequate hemostasis was obtained, closure was accomplished using 2-0 chromic to approximate deep tissues in an interrupted fashion and interrupted 4-0 Vicryl subcuticular stitch. Skin margins were reinforced with Steri-strips. The patient tolerated the procedure well without obvious complication and was taken to the recovery room in a satisfactory condition. All sponge and needle counts were correct. Estimated blood loss was less than 50 cc. There were no drains left in place. CPT code
