NFPE Mini Assignment (25 points) NUTR 3400 Assignment Directions: Read the case background below and answer the questions in complete sentences. Case Background: Jeremy is a 59-year-old male with a history of duodenal cancer. Three months ago, he underwent a Whipple surgery (see graphic) as part of his cancer treatment. After surgery, he experienced significant complications that prevented him from eating any foods by mouth. In response, he was put on enteral nutrition support but did not tolerate this either. During this time, he unintentionally lost 22 pounds over a 2-month time period. His intake only met ~40% of his requirements during this time. Jeremy’s usual adult bodyweight was 210 pounds. As a result of the surgery complications, Jeremy was placed on home total parenteral nutrition (TPN) one month ago. Since going on TPN, his weight has stabilized. However, when you visit with him you notice severe muscle wasting in his temporal region and squared off shoulders. You also note fat loss around the orbital region of his eyes. This week, Jeremy received word from his physician that his internal wounds have healed, and he can begin resuming intake by mouth. He has been cleared to begin consuming a full liquid diet this week, progressing to a pureed diet, then a minced and moist/ soft diet as tolerated. Questions to Answer (worth 3 points each unless otherwise stated): 1) Calculate his percent weight loss. 2) Calculate his percent usual body weight. 3) According to the AND/ASPEN criteria, does Jeremy meet the criteria for malnutrition? If so, what severity of malnutrition (using the malnutrition in the context of chronic illness category) does he have? Explain the criteria you used to make this malnutrition diagnosis. (5 points) 4) According to the GLIM criteria, does Jeremy meet the criteria for malnutrition? If so, what severity of malnutrition does he have? Explain the criteria you used to make the malnutrition diagnosis. (5 points) 5) What is a probable nutrition diagnosis for Jeremy? 6) What other nutrition focused physical examinations would you perform and why (be detailed as to what specifically you want to explore further for this patient and why that seems important)? 7) What aspects of your nutrition assessment (focusing on clinical assessment + NFPE) would you want to monitor and evaluate over time as Jeremy reintroduces foods into his diet and weans off of parenteral nutrition support?
