In this assignment we will continue to build onto your Patient-Centered program plan started in your Module 3 Portfolio Assignment. Include the following items in your submission: Consider the types of patients, family members and caregivers that will be part of your patient-centered care plan. Create a mini-biography of the 3 most common patients, family members, or caregivers you might encounter. Describe who they are, where they live, what support they have in their healthcare both financially and socially. Include information about their education, language, and experience with healthcare. For each patient, family member and/or caregiver, outline 2-3 barriers the patient may experience in understanding health information specific to your setting. For ONE of your 3 patients, provide a list 2-3 tools appropriate for your setting that could be helpful in overcoming those literacy barriers.
