In the past few days, she started getting sicker, feeling warm and shaking chills, and dizzy. She was seen on an outpatient basis and diagnosed with pyelonephritis. Outpatient versus inpatient treatment was discussed, however, the patient wishes to try outpatient. She called her physician today, indicating she was feeling sicker and the decision was made to admit her for further treatment. General: Fevers and chills HEENT: No complaints Cardiac: No complaints Respiratory: No complaints GI: No diarrhea Psychological: Quite anxious General, ill-appearing female. Vitals, temperature 101.2, pulse 88, respirations 24, BP 120/80. HEENT exam normal. Neck supple without adenopathy. Heart regular rate and rhythm. Lungs clear. Abdomen, soft with tenderness in lower quadrants. Pyelonephritis Admit patient, give her IV fluids, IV antibiotics. Continue IV antibiotics until afebrile for 24 hours or more DISCHARGE SUMMARY Patient admitted for pyelonephritis of several days duration. Attempted treatment as outpatient, however, symptoms continued to increase, therefore, she was admitted for further treatment. On Day 4, she presented with no complaints and her fever had resolved.
