62 yo female admitted from LTC for altered mental status, pt is confused and disoriented, and UTI. Pt with confusion x 1 week and foul-smelling urine. Pt is confined to wheelchair however can transfer to bedside commode with assistance. For past week, pt has refused to get out of bed and caregiver has allowed her to lay in same position. Noted with sacral pressure injury, stage 2, upon admission. PMH: s/p subdural hematoma, migraine, R LE DVT, obesity Ht: 64” Wt: 180 lb General Appearance: Neuro: Alert and oriented to person, confused GI: Abd soft, non-distended, obese Musculoskeletal: Unable to move all extremities, wheelchair bound Skin: Stage 2 sacral pressure injury, partial thickness skin loss, 4 cm x 5 cm, shallow crater Nutrition: Pt normally with good intake however is only completing ~25% of meals currently. Is on a Kcal restricted diet (1400 kcal) at LTC, diet order is continued in acute care. Provide a PES statement
