Read the case study and answer the following questions: Micah and Sarah, best friends and colleagues, are staff nurses on the night shift in a pediatric surgical unit. One night, Micah happens to notice that a 6-year-old diabetic patient recovering from minor surgery looking very pale and was perspiring. Testing results confirms their concern: The child is extremely hypoglycemic. Micah calls the resident and assists in stabilizing the patient. The child recovers and was discharged home 2 days later. The next day, reviewing the incident with Simmons, Micah states how surprised they were to find the child hypoglycemic. At first, Simmons did not say much, but finally admitted mistakenly giving the child too much insulin. Simmons had not gone back to check on the child and realized the mistake only when Micah found the child unresponsive. Micah is surprised and asks whether Simmons had completed an incident report or notified the child’s physician afterward. Simmons says “No” because reporting the mistake would create an inquiry and undoubtedly make trouble. “I can do without that right now,” Simmons says. Simmons explains that they are being reviewed for promotion to the next step in the clinical ladder program of the nursing division and fears that the incident would prevent promotion and the financial benefit it would bring. Simmons looks at Micah and says intently with tears in their eyes, “Please don’t report this. I only told you in confidence because you are my friend. It was just a simple mistake, and the patient recovered. It will never happen again. Please promise me you won’t report this.” Micah promises to keep the secret, but soon has second thoughts. Given their workload, it’s not surprising that such a mistake occurred, and certainly there was no malice or gross irresponsibility involved – Simmons was not drunk or thoughtless in her work. But while understandable, Simmons’ mistake was a serious one. If Micah had not noticed the child’s condition, the child could have lapsed into a coma and suffered a variety of physical injuries and may well have died. Simmons is Micah’s best friend and Micah promised to keep the secret, but Micah wonders if, despite this, they have a duty to report the error. Should Micah report the error? Questions 1. What are the relevant facts? 2. What are the ethical issues? For each test, give a brief account of the philosophical reasoning behind the test. Act Utilitarianism 1. What good and bad outcomes can be expected? 2. What would promote the greatest utility for the greatest number of people? Rule Utilitarianism 1. Would this action, if it were a generally followed social policy, promote the greatest long-term utility compared to alternative policies? 2. What would be the best policy for this profession to follow in the long run?
