: Part-A “Health assessment refers to a systematic method of collecting and analysing data for the purpose of planning patient- centered care. The nurse collect health data from the patient and compare these with the ideal state of health, taking into account the patient’s age, gender, culture, ethnicity and physical, psychological and socio-economic status. Data about the patient’s strength, weaknesses, health, problems, and deficits are identified (Wilson & Giddens, 2016, p. 1). “An assessment is a clinician’s evaluation of disease or condition based on patient’s subjective report of the symptoms and course of the illness or condition, and the clinician’s objective findings. These findings include data obtained through laboratory tests, physical examination and medical history; and information reported by carers, family members and other members of the health care team.”(National safety and Quality Health Service Standards,2021, p. 75). “Five core competencies identified by the Institution of Medicine are essential for all health care professionals to demonstrate in all areas of practice. These include 1) provide patient-centred care, 2) work in interdisciplinary teams, 3) use evidence based practice, 4) apply quality improvements, and 5) use informatics” (Wilson & Giddens, 2016, p.1). For example, a patient might mention specific sources of stress or allude to a family history of a particular condition. It is essential to recognise that assessment is an ongoing, dynamic process. Nurses continuously collect data, assess patient responses, and adapt care plans as the patient’s condition progresses. This proactive approach ensures that care remains responsive and tailored to evolving patient need (Bowen, Draper, & Moore, 2025, p. 472). In essence, the nursing assessment is a comprehensive approach to understanding a patient’s health status. As (Weber and Kelly,2017, p. 2) describe, “The acute care nurse performs a focused assessment, and then incorporates assessment findings with a multidisciplinary team to develop a comprehensive plan of care”. The National Safety and Quality Health Service (NSQHS) Standards, established by the Australian Commission on Safety and Quality in Health Care emphasize the importance of high-quality care and safety in healthcare settings. Amongst these, the Comprehensive Care Standard is particularly relevant to health assessment. “The Comprehensive Care Standard aims to ensure that patients receive comprehensive health care that meets their individual needs and considers the impact of their health issues on their life and well-being. It also aims to ensure that the risks of harm for patients during health care are prevented and managed through targeted strategies Comprehensive care is based on identified goals for the episode of care. These goals are aligned with patient’s expressed preferences and health-care needs, consider the impact of the patient’s health issue on their life and well-being, and are clinically appropriate. (Australian Commission on Safety and Quality in Health Care, 2021, p. 75). .” (Australian Commission on Safety and Quality in Health Care, 2021). Action 5.03 under the Comprehensive Care Standard outlines the following essential practices: 1.Actively involve patients in their own care, 2. Meet the patient’s, information needs, and 3. Facilitate shared decision-making (Australian Commission on Safety and Quality in Healthcare, 2021, action 503) Part-B According to (Nursing and Midwifery Board of Australia, 2016, P. 4) “RN practice is based on purposely engaging in effective therapeutic and professional relationship. This includes collegial of generosity in context of mutual trust and respect in professional relationship”. “In therapeutic relationship, the nurse is sensitive to a person’s situation and purposefully engages with them using knowledge and skills in respect, compassion, and kindness in the relationship. The persons right and dignity is recognised and respected” is stated in glossary by (Nursing and Midwife Board of Australia, 2016, P. 7). “Additionally, the nurse is vital for ensuring that information transfer occurs within multidisciplinary team communication with other care team member is professional, organised, accurate, complete, and concise” (Ernstmeyer & Christman, 2021, p. 84). It Is essential to consider how both verbal and non-verbal communication are aligned with standard 2.2. Verbal communication The aim of the verbal communication is always to speak with clarity, accuracy, and honesty. It is also essential to know your audience and speak appropriately according to patie
