A 55-year-old man arrives at the emergency department following a motor vehicle accident. The patient is hemorrhaging from a lacerated spleen and requires emergency surgery. Pretransfusion testing determined that the patient’s phenotype is group A, D-negative with a negative antibody screen. Crossmatches with RBC units were compatible by the immediate-spin crossmatch. During surgery, the patient receives six units of group A, D-negative RBCs and four units of group A frozen plasma. Three days later, during the first 15 minutes of a subsequent RBC transfusion using a blood-warming device, the patient developed fever and chills.
