Mrs. McDonald comes into the pharmacy complaining of a headache and dizziness. You look at her patient profile on the computer and notice that she started a new blood pressure medication a few days ago. You remember her coming into the pharmacy that day during a very busy time in the pharmacy. She had patiently waited for her prescription since her daughter was waiting in the car to drive her home, which was her only means of transportation that day. You ask the pharmacist to look at Mrs. McDonald’s file and the pharmacist notices that a warning flag was dismissed when Mrs. McDonald’s new prescription was dispensed. The warning message stated that there was a “therapeutic duplication” since Mrs. McDonald was already taking a medication for blood pressure. You try to remember dispensed her prescription that day and do recall dismissing that warning message and not notifying the pharmacist since it was really busy. The pharmacist now questions Mrs. McDonald about the use of her new medication and she states that she started taking the new medication as prescribed but did not discontinue her previous blood pressure medication because no one told her to. The pharmacist measures her blood pressure and it currently is 101/64 mmHg which will explain the headache and dizziness she is currently experiencing. How do you prevent an error like this from occurring in the future? How important are warning messages?
