Presentation: Mrs. C is a 72-year-old female presented to the emergency room after a fall at home. Client reports that she did not lose consciousness when she fell. Client’s chief complaint: “I fell in the middle of the night. I was getting up out of bed to go to the bathroom.” Past medical history: Gallbladder removal at the age of 45. Takes Tylenol on rare occasions for mild headaches. Social history: Client’s spouse died 3 months ago, and now lives alone. She is Catholic and attended religion services weekly up until her spouse died. Client reports that she does not have any living family members and drives to the local senior center weekly to socialize. Mrs. C sees her primary care provider for her annual checkups. Admitting vital signs in the emergency room: T: 100.4, BP 112/78, HR 80, RR 20, O2 sat: 98% on room air Nurse’s assessment: Client’s right wrist is slightly swollen with a small purple hematoma. Client rates 3/10 pain to the right wrist area, but only when she tries to move it. When asked to move the right wrist, client grimaced. Client also has a 0.5 cm x 1 cm laceration over the right eye that is not currently bleeding and is without erythema or pain when the area is palpated. Client is following commands and is oriented to person, place, and time. Her pupils are equal and reactive to light and accommodation, her speech is clear and appropriate, she has no evidence of a facial droop, tongue is midline, and her upper and lower body strengths are equal and strong. Skin turgor is poor with notable 2+ (moderate) tenting on her forearm. Last bowel movement was 2 days ago. Abdomen is soft, nontender, nondistended. Client has no complaints of dysuria, urgency, or frequency.
